In June 2025, a Boeing 787 Dreamliner on Air India Flight 171 crashed shortly after takeoff from Ahmedabad, India, en route to London. 241 passengers and crew members perished on the spot, alongside 38 additional fatalities on the ground. Now, more than half a year after the deadly incident, new and troubling details are emerging about the sequence of failures that led to one of the most severe aircraft accidents in recent years.
An Ambiguous Interim Report and a Game-Changing Document
The official investigation by Indian authorities has yet to conclude, but an interim report published in July 2025 hinted that the pilots might be responsible for the accident, as the fuel system switches moved from “on” to “off” shortly after takeoff. Various theories emerged on social media and in the media, including speculations about pilot error or even suicidal intent. However, a thorough examination did not reveal any medical or behavioral findings indicating such intent, and families demanded answers.

It was also noted that in April 2022, the aircraft was grounded for an extended period due to an issue with the landing gear indication system – a malfunction that required the replacement of several modules.
Call for Criminal Investigation in the U.S.
The FAS report generated widespread public attention and led to calls for a criminal investigation against all involved parties, including Boeing, Air India, and Indian regulators. The Indian newspaper Hindustan Times reported that according to FAS, passengers on Dreamliner flights might be exposed to safety hazards that have not been disclosed to the public.
The organization directly approached the U.S. National Transportation Safety Board (NTSB) and the Federal Aviation Administration (FAA), demanding the disclosure of all deficiencies and a comprehensive inspection of the entire active Dreamliner fleet.
Response from Parties: Silence and Disregard
Following the publication, Boeing stated that it would work with the Indian authority according to ICAO’s international protocols (Annex 13), but did not address the content of the report itself. India’s Civil Aviation Ministry refused to comment, and Air India did not provide any response.
